The Best Heating Pads for Hip Pain

Hip pain is the opposite buying problem to knee pain. A knee needs a small pad that keeps contact on a joint that bends; a hip needs area, because the thing that hurts runs from the bony point on the side, back through the glute, and up into the lower back. The pad to buy is the biggest one that still has an auto shutoff.

The short version

For most people with an aching hip, buy the Boileau XXXL. The reason is unglamorous: hip pain almost never sits in one patch, and this is the one pad large enough to put heat on the lateral hip, the glute and the lumbar spine at the same time instead of making you choose. If you want to spend less, the PureRelief XL is the standard 12 by 24 in pad and it is plenty for one side. If your hip hurts most when you are up and about, the Comfytemp cordless is the only one you can wear while you move. The shaped neck pad is the wrong tool here, and the more important caveat is at the bottom: for the commonest kind of hip pain in people over 40, heat is the warm-up, not the treatment.

Why a hip is the opposite problem to a knee

We ranked heating pads for a knee on contact: a knee is small, bony and bends, so a big flat pad bridges over the kneecap and heats the air underneath. A hip inverts every one of those facts. It is deep — the joint itself sits several inches inside a cone of muscle — and what people call hip pain is usually a region, not a point: the greater trochanter on the outside, the gluteal muscles behind it, the hip flexors at the front of the crease, and the lower back that always joins in. There is no shaped pad for that. There is only area.

Depth also sets expectations. Surface heat from an electric pad warms skin and the tissue immediately under it. It is not reaching the joint capsule of a hip through that much muscle, and no pad on the market does. What it reaches is everything on the way in, which is where a good deal of the discomfort lives anyway, and warming that makes the hip feel more willing to move. That is a real and useful effect. It is just not a treatment for the joint.

How we ranked these for a hip

  1. Area first. The winning pad is the one that covers hip, glute and low back without being repositioned. On a hip, a bigger rectangle genuinely is better — the opposite of our advice for a knee.
  2. Auto shutoff, no exceptions. Hip pain is an evening and overnight complaint, which means people fall asleep on these. A pad without a timer that cuts the element does not get ranked.
  3. Does it stay put in the position you hurt in?Side-lying, lying on your back, sitting, or walking about — each favours a different pad. Weight helps lying down; cordless is the only thing that works standing up.
  4. Moist-heat option, where offered. Not magic. It does make the heat feel like it is getting further in rather than sitting on the skin.
  5. Washable cover and real cord length. The difference between a pad you keep using and a pad that lives behind the sofa.

Hip and the lumbar spine togetherBoileau XXXL, or the weighted pad while seatedHip and low back pain travel together. Treating both at once is the main argument for the big pad.
Pain on the bony point when you lie on that sideHeat before movement, then read the loading section belowThat pattern is usually gluteal tendinopathy, and tendon problems respond to graded loading. Heat makes the session tolerable; it does not do the session.

What the evidence actually says about heat

No randomized trial has tested an electric heating pad on a hip, and we are not going to pretend otherwise. The nearest evidence is on other joints, and it is mixed. The Cochrane review of thermotherapy for osteoarthritis pooled three randomized trials in 179 patients with kneeosteoarthritis and found that hot packs had no beneficial effect on swelling compared with placebo or with cold, while the one clinically important result in the review belonged to ice massage — a 29% relative improvement in quadriceps strength over three weeks. The companion Cochrane review of thermotherapy for rheumatoid arthritis treats heat as a safe palliative adjunct rather than a disease treatment, which is the honest job description.

So the case for a heating pad on a hip is comfort and willingness to move, not repair. That is worth fifteen dollars and twenty minutes before a walk, before stairs, or before the strengthening work that does move the needle. It is not worth skipping the strengthening work for.

The loading half, which a pad does not replace

Lateral hip pain — the ache on the bony point that is worst lying on that side and climbing stairs — is most often gluteal tendinopathy rather than “bursitis”, and tendons answer to graded loading. The same goes for the glute and piriformis pattern that refers down the back of the thigh. Heat is a legitimate way to make that work tolerable; it is not a substitute for it. Our tight hips page covers what to actually do, and glutes and piriformis covers the referred kind.

Where a heating pad is the wrong purchase

If the pain is nerve-flavoured — burning or shooting down the leg, worse at rest — electrical stimulation is the better-studied home option and the TENS roundup for hip pain is the page for it. If what hurts is muscle rather than joint, a gun reaches further than heat does: see the best massage guns for glutes and hips. If both legs feel heavy and swollen at the end of the day, compression boots are the category. And if you are treating a lower back more than a hip, the general heating pad guide ranks for that span instead.

Boileau XXXL Electric Heating Pad (17 × 33 in)

Large enough to cover a whole lumbar region and both sides of it at once, which a standard pad cannot do — and area is the only heating-pad spec that changes the experience.

What it gets right

  • 17 × 33 in covers the full lumbar span rather than a patch of it
  • Multiple heat settings with auto shutoff

What it does not

  • Bulky to store
  • Takes longer to reach temperature

Pure Enrichment PureRelief XL Heating Pad (12 × 24 in)

The default answer in this category: standard 12 × 24 in footprint, multiple heat settings, auto shutoff, and a washable cover — nothing clever, everything you actually need for a back.

What it gets right

  • 12 × 24 in is the size that spans a lower back rather than treating a patch of it
  • Auto shutoff, which is the one feature that matters if you fall asleep on it
  • Machine-washable microplush cover

What it does not

  • Corded, so you are treating your back wherever the outlet is
  • No weight to it, so it lifts away from the curve of your back when you sit upright

Comfytemp Cordless Heating Pad with Massager

Battery heat plus vibration, aimed at people who want to walk around while treating a back — which is the one thing a sore lower back is usually least interested in doing.

What it gets right

  • No cord, so it works in a car, at a desk away from a socket, or standing at a counter
  • Adds vibration on top of heat, which some people prefer to heat alone
  • Listed as FSA and HSA eligible

What it does not

  • Runtime per charge is the limiting factor for a 20-30 minute session; check the current listing figure before buying for that
  • Costs more than a corded pad of the same size
  • A lower back is usually treated sitting or lying down, which is where a cord is not a problem

Comfytemp Weighted Heating Pad (12 × 24 in)

The weight is the whole point and it is not a gimmick: a weighted pad stays in contact with the curve of a lower back when you are sitting, which is exactly where an unweighted pad lifts off.

What it gets right

  • Weighted, so it holds contact against a lumbar curve instead of tenting away from it
  • Listed as FSA and HSA eligible, which changes the real price for a lot of buyers
  • Auto shutoff and a washable cover

What it does not

  • The weight is the reason to buy it and the reason some people dislike it on a sore back
  • Heavier to hold in place anywhere other than a back
  • Corded

Sunbeam Neck and Shoulder Heating Pad

Cut to the shape of the problem: it sits over the trapezius and down both sides without being held there, which is the only thing a flat rectangle cannot do on a neck.

What it gets right

  • Shaped for the neck and shoulders, so it stays put without a hand on it
  • Moist-heat option as well as dry
  • Auto shutoff — the feature that matters because people use this one in an armchair in the evening

What it does not

  • The shape that makes it good on a neck makes it useless on a lower back
  • Corded, so you are anchored to the outlet
  • Covers less total area than a flat XL pad

Common questions

Is heat or ice better for hip pain?

Heat for a stiff, achy hip you are about to move; ice for a hip that is hot, swollen or freshly aggravated. The honest caveat is that the trial evidence here comes from knees, not hips: the Cochrane thermotherapy review found hot packs had no beneficial effect on swelling versus placebo or cold, while ice massage produced the one clinically important result in the review. For the common stiff-hip presentation, heat before movement is still the sensible cheap option.

What size heating pad is best for a hip?

Bigger than you would buy for a knee. A hip complaint usually spans the bony point on the outside, the glute behind it and the low back above it, so a 17 by 33 in pad covers in one go what a small pad needs three positions to reach. A standard 12 by 24 in pad is enough if you are treating one side only. A pad shaped for a neck and shoulders is the wrong object entirely.

Can a heating pad help hip bursitis?

It can make it more comfortable, and it is not treating it. Pain on the bony point of the hip that is worst when you lie on that side is now usually understood as gluteal tendinopathy rather than inflamed bursa, and tendinopathy responds to graded loading over weeks. Use the heat to make that work and the evenings tolerable, not instead of the work. If the hip is hot, red and swollen with a fever, stop and get it looked at.

Does a heating pad reach the hip joint itself?

No. The hip joint sits deep inside a cone of muscle, and surface heat warms skin and the tissue just beneath it. What a pad genuinely warms is everything on the way in — glutes, lateral hip, lumbar muscle — which is where a lot of the discomfort lives anyway. Any product claiming to heat a joint capsule through that much tissue is selling you something.
