Best Tools for Lower Back Pain

The most-searched problem in this whole category, and the honest answer is the cheapest device on the site. Heat suits a guarding muscle in a way percussion actively does not — and there is one popular technique here that we would tell you to stop doing.

For a generally stiff, aching lower back with no radiating symptoms, a large heating pad is usually the right first purchase — cheaper than any massage gun, passive, and better suited to a muscle that is guarding. Add a peanut-shaped ball for the muscle either side of the spine. Do not put a single foam roller under your lumbar spine.

What is going on, without diagnosing you

Most non-specific lower back pain involves muscle that has tightened and is holding protectively, whether or not something structural started it. Two mechanically important facts follow.

First, the lumbar spine has no bony protection behind it the way the thoracic spine has a rib cage. The bones themselves are close to the surface. Anything you press into your lower back needs to land on the muscle either side of the spine and not on the spine.

Second, a muscle in spasm is already over-stimulated. Adding percussion to something contracting involuntarily commonly makes it guard harder. That is the whole reason heat outranks a massage gun on this page.

The mistake to stop making

Lying on a single foam roller placed under the lower back is extremely common and it is not a good idea. The roller contacts the lumbar spine directly, and it also encourages the lower back to extend over the roller — which is the opposite of what most irritated lower backs want.

A foam roller across the upper back is a different and genuinely useful thing: the rib cage protects the thoracic spine and the roller works as a fulcrum for extension. Upper back, yes. Lower back, use a peanut-shaped ball so the gap straddles the bone.

The sequence that actually works

  1. Heat, 15–20 minutes. Long enough to warm tissue rather than skin.
  2. Move gently for two or three minutes. This is the part everyone skips and it does most of the work. Heat opens the door; movement walks through it.
  3. Then pressure, if the muscle will tolerate it.Peanut ball either side of the spine, 60–90 seconds, or a low-setting gun with a fork attachment.
  4. Then gentle range work — not aggressive stretching into pain.

What the evidence actually supports

We are not aware of research establishing that any device on this page treats the cause of lower back pain, and we are not going to imply otherwise. What is documented for percussion generally is short-term reductions in tissue stiffness and improved range of motion in healthy participants, and a comparison against foam rolling found neither significantly reduced pain more than rest.

Which is worth holding onto: these are comfort and mobility tools. For most non-specific lower back pain the interventions with the best support are movement, gradual loading and time — none of which we sell.

Common questions

Is a heating pad or a massage gun better for lower back pain?

Heat, usually. A muscle that is guarding or in spasm is already over-stimulated, and adding percussion commonly makes it guard harder. Heat is passive input that reduces stiffness without demanding anything of the tissue. Once the acute guarding settles, a low-setting gun with a fork attachment on the muscle either side of the spine is reasonable.

Can you foam roll your lower back?

Not with a single roller. The lumbar spine has no bony protection behind it, so a roller contacts the bone directly, and it also pushes the lower back into extension — the opposite of what most irritated lower backs want. When should I see someone about back pain?

Urgently for numbness around the groin or inner thighs, changes in bladder or bowel control, weakness in a leg or foot, or pain after significant trauma. Soon for pain radiating below the knee, unexplained weight loss, fever, night pain that wakes you, or a history of cancer. And anyway if it has not improved in several weeks.
