Frozen Shoulder

A frozen shoulder is a capsule problem, which is why it loses range in every direction rather than hurting in one. The meta-analysis is unusually blunt about what helps: exercise and stretching improve range, function and pain, supervised work beats doing it alone, and adding physical modalities on top has no beneficial effect. That makes the honest shopping list very short.

The thirty-second version

If your shoulder has lost range in everydirection — overhead, out to the side, and reaching behind your back to a bra strap or a back pocket — and it came on without a single clear injury, you are describing the pattern clinicians call frozen shoulder, or adhesive capsulitis. The treatment with evidence behind it is exercise and stretching, ideally supervised. The single most useful thing to buy is a non-elastic stretch strap, because it makes the range-of-motion work possible without a second person. Almost everything else sold for this is comfort, and the meta-analysis is unusually direct about that.

Why it loses range in every direction

The shoulder joint sits inside a capsule: a sleeve of connective tissue that holds the joint fluid and allows the ball to travel inside the socket. In a frozen shoulder that capsule thickens and contracts. The consequence is mechanical and it explains every symptom. A tight sleeve limits rotation first, which is why reaching behind your back goes before reaching overhead does, and why the shoulder feels like it hits a wall rather than hurting through a range.

That mechanism is also why the tools this site covers can only ever play a supporting role. A massage gun works on muscle. A capsule is not muscle, it is not reachable through the deltoid, and no handheld device loosens connective tissue inside a joint. What muscle work does do is settle the traps, deltoid and pec that have been compensating for months, and that is worth something — just not what the marketing implies.

What the meta-analysis actually found

The 2022 systematic review and meta-analysis in Archives of Physical Medicine and Rehabilitation screened 33 studies and pooled 19. Its findings, in its own terms:

  • Range of motion, function and pain improve both with exercise alone and with programs that include exercise.
  • There was preliminary evidence that supervised exercise is more beneficial than home exercise for range of motion and function. That is the finding most worth acting on and the one nobody can sell you.
  • Adding stretches to a multimodal program may increase range of motion. This is the sentence the stretch strap on this page exists for.
  • Adding exercise improved active range of motion compared with a program without it, whereas adding physical modalities had no beneficial effect. “Physical modalities” is the bucket that heat packs, electrotherapy and ultrasound live in.
  • Muscle energy techniques came out as a beneficial type of exercise therapy for improving function compared with other exercise types, and multimodal programs showed little to no difference in range of motion against exercise alone.

So the honest hierarchy is: get a program, be supervised doing it, include real stretching, and do not expect the plug-in things to move the result. We sell plug-in things. We would rather tell you that than sell you one under false pretences.

Stretch strapBudgetThe one purchase the evidence points at: adding stretches to a program may increase range of motion, and a non-elastic strap with loops lets you hold a position at a set length on your own.Where it fails: Forcing range on an angry shoulder sets you back a week. It supports a program, it is not a treatment, and a belt or towel does most of the job for nothing.Start here
Massage gunMid-rangeUseful on the traps, deltoid and pec that have spent months compensating. A light gun you can hold with the good arm is the requirement.Where it fails: It cannot reach the capsule, which is the actual problem. Never over the joint line, the bony point of the shoulder, the armpit or the neck.Sometimes
Heating padBudgetComfort before a stretching session, and most people find a warm shoulder easier to move. Cheap, pleasant, low risk.Where it fails: The meta-analysis found adding physical modalities to an exercise program had no beneficial effect. Buy it to make the session tolerable, not to change the outcome.Sometimes
Massage ballBudgetA ball against a wall reaches the back of the shoulder and the pec one-handed, with no grip needed from the stiff arm.Where it fails: Pointless and painful on the joint itself or on the bony ridge of the shoulder blade.Sometimes
TENS unitBudgetSymptom relief for a painful evening in the early, most painful phase. Shallow tissue, so surface stimulation can reach it.Where it fails: Electrotherapy sits in the modality bucket the meta-analysis found added nothing. It does not loosen a capsule.Sometimes

What to look for, if you are buying for this

  1. Non-elastic, with loops. Elastic gives back the range you just gained. Sewn loops let you progress one notch at a time instead of guessing how far you went last week, which is the whole point over months.
  2. Usable with one hand and no grip. The affected arm cannot help much. If a tool needs two strong hands, it will not get used.
  3. A light gun, if you buy one at all.Weight decides whether you can hold it against your own shoulder. Power is irrelevant here — you are treating traps and pec, not trying to reach anything deep.
  4. Heat that stays put on a sloping shoulder. A flat rectangle slides off. The pads cut for neck and shoulders do not.
  5. Nothing that claims to release adhesions or break up a capsule. No home device does that, and a product that says it does is telling you how much to trust the rest of its copy.

What we would actually buy

One of these supports the thing the evidence points at; the other two make the months more bearable. We are putting them in that order rather than in order of price.

The one with a reason

OPTP Original Stretch Out Strap

Non-elastic with sewn loops, so an overhead or external-rotation stretch holds at the length you set rather than springing back. The meta-analysis found adding stretches to a program may increase range of motion - this is the cheapest way to add them on your own.

How to run the stretching, without setting it back

The stretches themselves should come from the clinician who assessed you — the review found supervised programs beat home programs, and the reason is that someone watching corrects the thing you cannot see yourself doing. What follows is how to use a strap sensibly within whatever program you are given.

Little and often beats long and heroic. Several short sessions a day at a tolerable stretch gets more range over a month than one punishing session that leaves the shoulder angry for two days. Warm first if it helps you. Fifteen minutes of heat before you stretch makes the session easier to tolerate; the evidence says it will not change where you end up, which is a reason to use it for comfort rather than to count it as treatment. Hold at length, do not bounce. The loops exist so that you can set a position and keep it, and so that next week you can go one loop further and know that you did. Expect a long timeline. This is a condition measured in months, and the thing that correlates with getting range back is having kept doing the work, not having bought a better device.

Where to go next

If the shoulder still rotates freely and only hurts reaching overhead, read rotator cuff paininstead — it is the more common problem and it is managed differently. If the pain is across the top of the shoulders and up the neck, that is neck and shoulder pain. For the lightest guns, which is the only specification that matters when the other arm is doing the work, the seniors roundup is the page; for the heat half, the shaped heating pad roundup. And if you are weighing a clinic route for a stubborn shoulder, massage gun vs shockwave therapy explains what the machine in the clinic does that your gun cannot.

Common questions

What is the fastest way to loosen a frozen shoulder?

There is no fast way, and the honest answer is a supervised exercise and stretching program done consistently over months. The 2022 meta-analysis in Archives of Physical Medicine and Rehabilitation pooled 19 studies and found range of motion, function and pain improve with exercise, that supervised exercise showed preliminary benefit over home exercise for range and function, and that adding stretches to a program may increase range of motion. Anything promising to free it up in days is describing something that has not been shown to happen.

Does a massage gun help frozen shoulder?

Not for the capsule, which is the actual problem. A frozen shoulder is a thickened, contracted joint capsule, and no handheld device reaches inside a joint. What a light gun can do is settle the traps, deltoid and pec that have been compensating for months, which makes the stretching session more comfortable. Keep it off the joint line, the bony point of the shoulder, the armpit and the neck.

Is heat good for a frozen shoulder?

It is good for comfort and it does not appear to change the outcome. The 2022 meta-analysis found that adding physical modalities — the category that includes heat packs, ultrasound and electrotherapy — to an exercise program had no beneficial effect, while adding exercise itself improved active range of motion. Use heat for fifteen minutes before you stretch because it makes the session easier to tolerate, and do not substitute it for the stretching.

How is frozen shoulder different from a rotator cuff problem?

Direction and character. A frozen shoulder loses range in every direction, and rotation goes first — reaching behind your back becomes impossible before reaching overhead does, and the shoulder feels like it hits a hard stop. A rotator cuff problem usually keeps most of its passive range, hurts on the outside of the upper arm, and is at its worst reaching overhead or lying on it. A clinician can tell them apart in a couple of minutes, which is worth more than a self-diagnosis before you commit to months of work.
